Active Together

Active Together is a cancer pre-habilitation and rehabilitation service funded by Yorkshire Cancer Research.

The Active Together programme helps people to prepare for their cancer treatment and recover from it afterwards.

This includes personalised support with physical activity and exercise, nutrition, and wellbeing support.

Active Together works with people to identify and achieve goals that matter to them, with the intention of supporting lifestyle changes that improve quality of life.

The programme is a collaboration between Barnsley Hospital and Barnsley Premier Leisure.

Active Together is delivered by a team of cancer specialists including physiotherapists, exercise professionals, healthy lifestyle advisors, dieticians and clinical psychologists.

How it works

Pre-habilitation Phase

After referral, patients are invited to attend an appointment to understand their needs, including assessments that measure aerobic capacity, strength, dietary, and psychological needs. Patients will then work with the team to create a personalised plan to help them prepare for their cancer treatment. This can include exercise classes, workshops, and 1:1 sessions.

During Cancer Treatment Phase

Support continues throughout treatment. Depending on each individual's treatment and needs, this may be over the phone, through group classes, or individual appointments.

Rehabilitation Phase

Following their treatment, patients will be invited to a follow-up appointment with the Active Together team so they can understand how best to support themselves moving forwards.

The team will support their rehabilitation for 12 weeks to meet their personal goals and achieve what matters to them, focusing on their physical, nutritional, and psychological needs.

This phase includes support at the Metrodome Leisure complex as required.

Supportive (After Rehabilitation) Phase

Active Together helps people to independently manage their physical and mental health in the future. They will be offered continued support and resources for a further 12 weeks after rehabilitation to enable long term lifestyle change and quality of life improvements.

How to refer

Referrals into the programme can be made by health professionals only. This must be upon diagnosis and only for people undergoing curative intent cancer treatment. This programme currently accepts referrals from the following tumour groups:

  • Colorectal
  • Lung
